Registration form for treatment

We’re delighted you’re here. We’ve talked about how it might be a good idea to start treatment with us, and as far as we’re concerned, everything is in place to move forward.

At the same time, we understand that this is an important step and that it needs to feel right for you too. So please do take your time to consider whether what we offer suits you and whether now is the right time to start.

You can easily register via this page if you decide to take the next step. As soon as we receive your registration, we’ll get in touch to discuss when we can start.

Please note! Don’t forget to enter your BSN number. Without your BSN number, we cannot claim the costs from your health insurance provider.
The registration form states that the application is still subject to assessment; in our case, we have already discussed your request for help, so this is no longer necessary.

Steps in the registration process

• 1. Complete the registration form.
• 2. Have the referral sent to us.*
• 3. Once we have received the registration form and the referral, we will provide you with a number of options for an initial consultation.

ZorgMail

You can find us on ZorgMail under ‘imhnederland@zorgmail.nl’. Referrers and/or healthcare professionals can use this platform to securely send the referral to us or contact us for consultation.

* The referring practitioner can send the referral directly to us via ZorgMail. Have you received the referral on paper? No problem – you can upload it via the registration form.

Would you like some more information about the referral? You can forward this page to your GP and/or current practitioner. It contains all the information they need to write the referral.
